When to Intervene on Acute Type B Dissection

Endovascular surgeon Grace Wang, MD, MSCE, FACS, examines time of intervention in patients with Type B (Debakey Type III) dissection, and reviews the management of this most common catastrophe of the aorta. Dr. Wang examines the pathophysiology of type B dissection, the temporal classifications for acute, hyperacute and subacute disease, high risk groups (including heritable aortopathies), management and treatment for both complicated (ruptured) and uncomplicated presentations, and considerations for emergent TEVAR. Includes a case review from a TEVAR procedure in a young patient with an extensive dissection, and a discussion of the relative benefits of TEVAR in the postoperative setting vs. the risk of morbidity and mortality.
